Management of Pulp Canal Obliteration in a Traumatized Maxillary Incisor using Cone-Beam Computed Tomography: A Case Report

Abstract

Pulp canal obliteration (PCO), a common consequence of dental trauma, complicates endodontic treatment due to canal calcification and altered internal anatomy. This case report describes the diagnosis and management of a maxillary right central incisor affected by PCO in a 17-year-old female patient with a history of dental trauma five years prior. Clinical and radiographic examination revealed crown discoloration, a non-vital pulp, and absence of a visible root canal on periapical radiograph. Conebeam computed tomography (CBCT) was employed to provide a three-dimensional evaluation, revealing canal obliteration with a patent segment in the apical third. Endodontic treatment was performed over two sessions under rubber dam isolation using magnification and ultrasonic-assisted irrigation. The canal was negotiated with small hand K-files (#6–15), the working length determined at 22 mm using an apex locator, and instrumentation completed with the RC Blue system (size 25/08) in reciprocal motion. The patient was asymptomatic postoperatively, and instructions were provided with a subsequent appointment scheduled for definitive restoration. This case underscores the importance of CBCT and modern endodontic techniques in managing complex calcified canals safely and effectively.
