Abstract

Objectives: Medication administration error is causing patient harm.  The study aims to review the interventions on and recommended interventions to medication administration errors in hospitals in Indonesia and Nigeria.

Methods: A mixed-method approach was employed to review available quantitative and qualitative studies.  Eligible studies were sought in Scopus and Embase/Medline.

Results: The review elicited two interventions that were conducted and fifteen recommended interventions.  The conducted interventions include medication safety practice training and pharmacist-led intervention in a hemodialysis room.  The most recommended interventions are education/training, having a medication administration error reporting system, proper drug labelling and packaging, provision of in-hospital clinical pharmacy service, and adequate staffing, among others.

Conclusions: There were identified conducted interventions on, and recommended interventions to, medication administration errors.  Based on context appropriateness, a combination of any of these interventions is recommended for the prevention, reduction, and elimination of medication administration errors.
