Navigating Across Hospital and Community Settings to Create a Patient Navigation Program for Older Adults

Abstract

Background: A co-created Patient Navigation Program (PNP) between a major acute care hospital and community agency was implemented to promote better integrated care for older adult patients transitioning from the hospital back to the community.

Audience: This workshop is for decision-makers, frontline clinical staff, older adults and family members, and researchers interested in patient navigation and older adult health.

Approach: The goal of this workshop is to showcase the development of PNP, which includes practice and system level facilitators and barriers faced by hospital and community providers (20 minutes), and an illustration of how implementation science was used to help guide patient navigation service delivery (5 minutes). Workshop attendees will be organized into small groups to reflect on patient, practice and system level issues impacting integrated care for older adults between hospital and community settings (20 minutes). A large group facilitated discussion framing the identified issues within an implementation science framework will be undertaken (5 minutes), which will be used by the workshop attendees to illustrate how their experiences with patient navigation have led to solutions to some of these issues (5 minutes). The workshop will end with reflections on key discussion points raised by workshop attendees with regard to what could be done to improve integrated care between hospital and community settings, and the perceived utility of implementation science for supporting this goal (5 minutes).

Outcomes: The outcomes of this workshop will provide attendees with knowledge regarding a PNP for older adults with complex care needs, the use of implementation science to critically reflect on factors facilitating or impeding integrated models of care, and a potential roadmap on how to work collaboratively across the care continuum.
