Abstract
An analysis of various reports concerning recent risks and crises which have escalated in the Swiss administrations shows that there has been a combination of failures in three, often over lapping, areas: organisation, leadership and information, communication and documentation. A crisis or a risk generally occurs when there is a chain of individual weaknesses throughout the three levels. Therefore it is essential to identify these weaknesses in everyday life and openly discuss and strive to correct them.
