Introduction/Background: Welcome to our world café workshop on integrated care, where experts, practitioners, and stakeholders come together to explore innovative approaches. Integrated care is more than just a buzzword, a transformative model that bridges gaps, enhances patient experiences, and optimizes health outcomes. In this dynamic workshop, we delve into key themes such as patient-centered approaches, interdisciplinary collaboration, and leveraging technology. Our goal? To foster meaningful dialogue, share best practices, and co-create solutions that transcend borders. Ideally, through the collaborative participation of a broad number of healthcare professionals, policymakers, patients, caregivers or advocates, your insights matter. Join us as we embark on this journey toward a more connected, compassionate, and effective healthcare system. Lets build bridges, break down silos, and pave the way for integrated care excellence.Why a workshop? A workshop facilitates participatory knowledge sharing, collaboration, innovation, policy alignment, and patient-centered care, all essential components for advancing integrated care practices is it for? The target audience for this workshop consists of (PhD) students, researchers, educators, patients and professionals in health and social care. What are we going to do?- 90. Introduction and background of the world cafe method and themes - (5)b. World cafe: we have three rounds of discussion and 4 table discussions.)- (3 x 20):. Patient-Centered Approaches: Discuss the importance of patient autonomy and shared decision-making. Explore case studies where patient-centered care has led to improved outcomes 2. Interdisciplinary Collaboration: Share examples of successful interdisciplinary teams in healthcare settings. Identify key factors that contribute to effective collaboration 3. Leveraging Technology in Care: Examine the role of technology in enhancing integrated care. Consider both the benefits and challenges of implementing tech solutions.. 4. Policy and Integrated Care: Debate the impact of current policies on integrated care practices and discuss what changes could be made to support better integration.c. Pitches and Conclusions, and general reflections (5). How are you going to engage with the audience? Everybody is an expert in her or his own research, education, practice or experiences. In small groups of 5 0 persons, we will discuss one of the four themes mentioned above. We challenge everyone to share their own experiences and ideas. How are you going to summarize the take home messages? All collected results will be collected and summarized by the organizing committee of this workshop. These results will be shared with the participants.
