Abstract
Introduction/Background: Integrated care is a holistic approach that aims to improve health outcomes by seamlessly coordinating services across different healthcare providers, settings, and disciplines. It recognizes that health and well-being are influenced by various interconnected factors, including medical, social, and environmental aspects.In the context of the Master Integrated Care Design program, we delve into the following key areas: Systems Thinking: Integrated care design requires a shift from siloed thinking to a systems perspective. We explore how to break down organizational barriers, foster collaboration, and create synergies among stakeholders. Patient-Centeredness: At the heart of integrated care lies the patient. We examine strategies to involve patients, families, and communities in decision-making, ensuring care plans align with their unique needs and preferences. Interprofessional Collaboration: Effective integrated care involves multidisciplinary teams working together seamlessly. We explore the dynamics of collaboration, communication, and shared decision-making. Technology and Innovation: Leveraging digital tools, data analytics, and telehealth, we explore how technology can enhance care coordination, improve outcomes, and empower patients. Policy and Governance: Integrated care design operates within a complex landscape of policies, regulations, and funding models. We analyze policy frameworks and governance structures to drive sustainable change.Join us in this exploration of integrated care design a journey toward a more connected, patient-centric, and resilient healthcare system.
Why a workshop?Using a workshop as a method to share values and perspectives in complex problems in integrated and personalized care. And how a master program can add on your already existing knowledge and experiences to get tools and different perspectives in solving complex problems.. Exchange of knowledge, expertise and experiences of all attendees is central in this workshop.
Who is it for?The target audience for this workshop consists of (PhD) students, researchers, educators, patients and professionals in health and social care.What are we going to do?- 90. In small groups of 5-0 persons we will discuss one of the four themes mentioned above. We challenge everyone to share their own experiences and ideas.How are you going to summarize the take home messages?All collected results will be collected and summarized by the organizing committee of this workshop. These results will be shared with the participants.
