Abstract
WellFort CHS, part of the Central West Ontario Health Team, developed the Lower Limb Preservation Initiative to reduce diabetes-related amputations in high-risk communities. Launched in 2023-2024, it aims to prevent many of Ontario’s ,200 annual amputations, which cost $40 million, by providing effective preventative care.
