Abstract
Integrated care thrives when patients need drive system design. Five years ago, we co-designed a program to address complex healthcare challenges. Our case collaborative initiative has helped over 220 patients and families, enhancing resource connection and care coordination. Providers and patients reported improved support, care quality, and empowerment.
