Abstract
The Health Innovation Group (HIG) builds capacity within Canadian national and provincial organizations to engage and support interdisciplinary teams, including patient advisors, across primary care, specialty care, and community settings. HIG supports teams in implementing systematic practice improvements that promote equitable access to integrated care for diverse patient populations. Through sustainable, principle-based practices, these Patient’s Health Home (PHH) Learning Expeditions address critical challenges such as access to continuity of care, integrated service delivery, cost reduction, and enhanced provider and patient experience. A joint commitment to organizational collaboration and continuous improvement positions these initiatives as leaders in transforming healthcare delivery.
This workshop is valuable for healthcare professionals, quality improvement leaders, government representatives, community organizations, patient advisors, researchers, and IT innovators dedicated to supporting sustainable, team-based improvements in integrated care delivery.
This interactive 90-minute workshop actively engages participants in hands-on learning and collaborative discussions. The session includes:
Introduction (10 minutes): A brief overview of HIG’s role and the impact of the PHH Expedition, including an introduction to the 9 PHH Implementation Elements and the Primary Care Sequence to Achieve Change.
Engagement with the PHH I-Compass and Toolkit (30 minutes): Participants will work with the I-Compass tool and implementation toolkit. In small groups, they’ll apply these resources to local scenarios, exploring each element and addressing specific challenges.
Breakout Discussions (20 minutes): Participants will break into interdisciplinary groups to discuss high-impact changes relevant to their settings (rural or urban), share insights, and tailor ideas to understand and meet diverse patient needs. Patient advisors in each group will contribute perspectives on designing integrated care approaches.
Case Studies (20 minutes): Highlights from five Canadian Practice Support Programs ( Palliser Primary Care Network, New Brunswick Medical Society, Newfoundland and Labrador Family Practice Renewal Program, Nova Scotia Health Services Practice Support Program) will showcase how teams are progressing and have achieved sustainable improvements with the PHH, including improved access and quality of patient care. Participants will reflect on these case studies and explore strategies for their organizations.
Action Planning (10 minutes): The session concludes with a Q&A and action planning. Participants will leave with draft action plans and strategies to implement PHH elements in their contexts.
Through these interactive components, participants will gain practical knowledge, engage in peer learning, and leave with a printed toolkit and actionable steps to drive sustainable improvements in their environments.
The workshop will demonstrate the value of interdisciplinary teams collaborating in PHH Learning Expeditions to improve healthcare delivery. Key engagement and recruitment strategies, as well as participant outcomes, will be highlighted. Participants will also actively use the Health Home I-Compass and implementation toolkit, gaining insights and tools to support sustainable improvements in their practices.
