The 10 "Sins" and "Wins" of Integrated Care

Abstract

Integrated care has been widely promoted as a solution for fragmented healthcare systems, especially for managing chronic conditions and enhancing outcomes for populations with complex needs. Yet, implementation comes with unique challenges. In this interactive workshop, participants will explore the ten 'sins', or common pitfalls, and 'wins', or successes, frequently encountered. Each 'sin' and 'win' aligns with the conference’s ten pillars of integrated care, creating a structured approach to examine real-life case studies for each theme. Ultimately, this workshop will build bridges, address gaps, and transform the complexities of integrated care into cohesive, patient-centred solutions, one step at a time.

Designed as an interactive, hands-on workshop, participants will explore the 'sins' and 'wins' of implementing integrated care models. The workshop introduces one pillar of integrated care at a time, with a real-life case study for each challenge. In small groups, participants will analyze pitfalls, brainstorm solutions, and reflect as a large group on additional challenges and opportunities to turn 'sins' into 'wins.'

For example, under the pillar of access, flow, and transitions, a common pitfall is creating more access points for unattached patients, potentially straining resources. A case study will illustrate this challenge, and participants will explore alternative solutions, such as building capacity in existing teams to balance access for attached and unattached patients.

Similarly, under the teamwork pillar, a common pitfall is prioritizing investments in clinical staff alone. A case study will highlight the value of incorporating unlicensed assistive personnel to enhance administrative and patient care.

Target audience: Any individual interested in exploring creative problem-solving approaches to implementing integrated care in practice.

Outcomes: At the end of this workshop, participants will learn how to side-step the most common pitfalls in implementing integrated care and how to leverage them to provide high-quality, patient-centred care in our complex and evolving healthcare landscape. 

Activities will be facilitated through tools such as Padlet and Mentimeter and key take-home messages will be summarized live for participants in a PowerPoint presentation. participants will be provided with workbooks during the session to support continued learning.

 

Workshop style:

Introduction (15 minutes): Overview of the ten pillars of integrated care and common pitfalls ("sins") versus successes ("wins") encountered during implementation.

Exploring Case Studies (45 minutes): Participants will review real-life case studies aligned with each pillar. In small facilitated groups, they will identify pitfalls and brainstorm solutions to transform challenges into successes, using interactive tools like Padlet and Mentimeter.

Reflection and Key Takeaways (20 minutes): Group reflections will summarize key lessons and strategies, with live updates captured in a summary document.

Q&A and Next Steps (10 minutes): The session concludes with a Q&A and draft action planning, equipping participants with practical strategies for integrated care improvements.
