Abstract
Background: Chronic diseases have long been managed through traditional self-management programs that often focus narrowly on disease-specific outcomes. This approach can overlook the broader context of individuals’ lives and the potential inherent in their unique skills and abilities. Our project seeks to address this gap by developing a 21st Century Digital model for Community Development that emphasizes empowerment, proactive health behaviors, and community support.
Approach: We will implement a multifaceted strategy that includes the promotion of entrepreneurial skills among patients to encourage innovative solutions for health management. Our approach involves creating a digital transformation toolkit designed for personalized health engagement, fostering collaboration among three local communities, healthcare providers, and NGOs. Community-based events, workshops, and support groups will be organized to enhance social connections and a sense of belonging. Additionally, interactive educational resources will be developed to inform individuals about secondary prevention and health condition management.
Results: We anticipate that this initiative will lead to improved engagement in health management among participants, increased social cohesion, and a strengthened sense of community. Early indicators of success will include the adoption of proactive health practices and enhanced well-being reported by individuals and informal caregivers. Continuous feedback will be gathered to monitor outcomes and refine the intervention strategies for greater effectiveness.
Implications: This project represents a significant shift in chronic disease management, moving from a disease-centric model to one that prioritizes individual empowerment and community collaboration. By nurturing a sense of purpose and value, we aim to cultivate proactive health behaviors that enhance overall quality of life. The findings from this initiative could inform future strategies for community-based health interventions, ultimately contributing to healthier, more resilient populations.
