Abstract
WellFort Community Health Services, in partnership with the Region of Peel (municipal government), and along with a provincial health funder, co-design and test a nurse practitioner-led, interprofessional, team-based approach to primary care, to better meet the health and social care needs of a growing homeless population. Learnings and recommendations for full implementation are discussed.
