Abstract

Purpose: 

Understanding the role that Integration plays in sustainable development by exploring how we put policy into practice. A space to identify solutions and think about how we collaborate to create a more sustainable and equitable future for our service delivery and patients.  For over 10 years a Links Worker Programme has worked across Primary Care services, bringing together the worlds of Primary Care and Third Sector. Throughout this period funding streams have changed, models of delivery have been reviewed, explored and cultivated. Governance and data collection is at the heart of ensuring that relationships across sectors can flourish. We will share our models that encourage interdisciplinary working and ensure it is embedded which promotes accountability and best practice to support patient care.

Key Themes:

  • Setting the context in Scotland- Health and Social Care Integration, Primary Care Improvement Plans, Social Prescribing and the Community Links Worker Programme.
  • What’s working in practice- highlighting key findings from 10 year anniversary, collaborative working across sectors as well as including areas of progress and gaps in implementation and delivery.
  • Solutions for the future- equipping communities to support a more sustainable and equitable future.

Audience: 

  • People with an interest in collaborative conversations about the future of health and care.

Approach: Two short presentations, (20 minutes) with time for participants to reflect, discuss and network through facilitated discussions (30 minutes).  Feedback and close (10 minutes)

Outcomes:  

  • Participants will have increased knowledge of Primary Care delivery in Scotland and the Health and Social Care Partnerships (HSCP) landscape.
  • Participants will have gained examples of good progress as well as understand the challenges/gaps across Third Sector and HSCPs in Scotland.
  • Participants will have greater understanding of the models of delivery used and how they have adapted in the last 10 years.
