Abstract
How can health systems move beyond theory to implement truly integrated, people-centered care at the district and community levels? Join us for a dynamic panel discussion exploring the practical entry points, tools, and adaptive strategies needed to introduce Integrated People-Centered Health Services (IPCHS) as part of a Population Health Management (PHM) approach.
Drawing on diverse experiences from public health leaders, community organizers, policymakers, and data innovators, this session will examine how integrated community care (ICC) can be tailored to reflect local priorities, resources, and lived realities—while staying aligned with the Triple and Quadruple Aims of better care, improved health outcomes, reduced costs, and provider well-being.
