Creating the Conditions for Integration: Bridging Evidence, Policy, and Practice for Transformative Healthcare

Abstract

Ireland is undergoing a significant radical transformation in healthcare with the implementation of Sláintecare and Health Regions. This transformation involves a shift towards integrated service delivery and "new ways of working." The HSE Organisation Development & Design (OD&D) team plays a crucial role in strategically and operationally working with services to create the conditions for change and integration through the implementation of the Health Service Change Framework, OD&D developmental pathway, tools, and resources. This workshop, framed within the Health Services Change Guide, showcases two studies that demonstrate services at national and operational level creating the conditions for change and integrated care.

Problem Statement: The healthcare system in Ireland is undergoing a profound transformation with the implementation of Sláintecare and Health Regions Implementation. There is a pressing need to effectively support this change, continuing the move towards integrated care aligned with people's needs. Participants will gain insights into the practical application of the Health Services Change Guide, the formal policy for change within the health services. The theme of the workshop is to demonstrate the journey from evidence to policy and policy to practice. The participants will gain insights into practical approaches that support integrated care and the realisation of Slaintecare delivering "right care, right place,right time”.

Workshop Objectives:

•Explore the Health Service Change Framework and OD&D Developmental pathway to build capacity for change and integration across the Health Service Executive in Ireland .Share two case studies illustrating change management in integrated care.

•Engage participants in a people and culture change approach to managing change in integrated care.

Target Audience: Professionals and stakeholders interested in creating conditions for managing change in integrated care using a people and culture change approach.

The workshop aims to provide insights into managing change in integrated care using a people and culture change approach. It targets participants interested in understanding and implementing this approach within their healthcare settings.

Participants will be engaged through Menti Polls, presentations, interactive and engaging liberating structure activity, and a visual map of observations/collage. 

Take-Home Messages: The workshop will summarise key insights through a collage and Menti poll output, emphasising the practical application of integrated care principles and the importance of a people-centred approach to change management. Participants will leave with actionable strategies for implementing integrated care within their respective healthcare contexts
