International case studies of integrated models of chronic disease care at the primary-secondary care interface

Abstract

Introduction: Better management of chronic disease care at the primary-secondary care interface is pivotal in managing the challenge of increasing health care demand in the context of an ageing population. However, compared to the substantial body of research and policy literature exploring international best practice in chronic disease care more broadly, there has been more limited effort to examine integrated models of chronic disease care at the primary-secondary care interface.

Context: Although the project initially responds to a UK-focused request, the search for good practice integrated models of chronic disease care at the primary-secondary care interface is a global need.

We consider “interface” as any form of interaction between the primary and secondary care sectors. While there are multiple examples of short-term management of acute escalations and transactional events and excellent micro-experiences, these are mostly too context-specific and/or too dependent upon exceptional individuals and exceptional circumstances. However, there is little knowledge of comprehensive experiences that emerge from a coherent system of integrated care, with coordination sufficiently embedded and flowing from an integrated care vision. There is a need to identify and assess the service delivery arrangements and various coordination mechanisms being deployed (e.g. existence of pathways; coordinated joint planning of care; established interdisciplinary teamwork; specific care coordination roles; availability of shared care records; technology supports coordination of care between professionals; etc).

This workshop will be of interest for everybody at ICIC24.

Structure

  • Introduction: presentation of the research project, UK NHS context, goals – Health Foundation [5 minutes]
  • Research methodology: How was the research conducted, country/case study selection criteria - IFIC [5 minutes]
  • Results: brief presentation of the case studies – IFIC [10 minutes]

oSummary-fiches of case studies will be distributed among participants, so they can have basic information at all times during the workshop. 

  • Discussion Tables with representatives from the case studies – participants are free to choose the case study of their interest [30 min]
  • Back to main group: Open discussion: [25 minutes]
  • Presentation of research results (relevance from the UK perspective): Health Foundation [10 minutes]
  • Closing: IFIC [5 min]

Engagement with the audience: Workshop participants will be able to choose the case study of their interest and participate in the Discussion Tables with representatives from the case studies. Participants will be able both to learn from the case studies and to share their own experiences. 

Take home messages: The results of the research will be published in the THF and IFIC websites.
