Abstract
The All Inclusive Care for Older People's (ALICE) program was initiated in July 2022 with the intention of developing a high-impact community-embedded model of care to support older people living in high-need communities on the Central Coast, Australia.
The emerging ALICE model is based on three principles: integrated care delivery, in which multidisciplinary teams from different health, aged and social care providers are connected at a local level to identify, assess and meet the needs of older people in a holistic way; connected communities that are empowered to leverage social connections, networks and organisations to support older people to stay active and engaged; and healthy place-making, which seeks to create a built and natural environment that supports older people in living active, healthy lifestyles.
To-date, the ALICE program has been focusing on understanding the needs of its community, undertaking a situational analysis to meet such needs, supporting a detailed co-design process of the emerging care model, and developing a benefits realization case.
This includes:
- a joint strategic needs assessment to identify priority neighbourhoods for the ALICE program;
- a resource pack, examining the evidence for the impact of the components of ALICE, including international case exemplars;
- a benefits realization model, demonstrating potential net total quantifiable benefits by Year 5 of $29.8 million with an additional $33.6 million in societal benefits (QALYS);
- the development of ongoing co-design forums with older residents on the Central Coast and a range cross-sectoral professional, organisational and community groups.
Using the 'Implementation Model for Integrated Care' [1], this case study paper will describe and reflect upon this journey and the next steps in the process that seeks to move from the planning and design phases to pre-implementation.
