Abstract: In response to the evolving landscape of home care services, Reykjavik City has undertaken a transformative initiative focusing on the seamless integration of digital solutions. Over many years, Reykjavik City's home care has been characterized by its integrated approach, where home nursing and social service teams collaborate closely. In recent years, our focus has expanded to enhance service accessibility, with specialized teams delivering acute services at home, the implementation of digital solutions, and a heightened emphasis on rehabilitating individuals to improve their quality of life and extend their ability to reside at home.
Introduction/Background: Reykjavik City's home care has long operated as an integrated service, with home nursing and social service teams working collaboratively. Recent efforts have concentrated on further improving service accessibility, implementing digital solutions, and enhancing rehabilitation strategies for individuals living at home. As we approach a critical juncture in our development, our attention turns to the seamless integration of these diverse components to provide tailored, high-quality care to individuals at home.
Target Audience: This presentation is tailored for professionals and industry participants in the home care sector, including healthcare providers, policymakers, and technology developers.
Involvement and Engagement: Our ongoing project engages key stakeholders in the design, implementation, and monitoring phases, emphasizing Personal and Public Involvement (PPI) and co-design approaches. This ensures that the integration process is shaped by the insights and needs of both service recipients and providers.
Initiative/Intervention: The project, set to conclude before the conference, systematically analyzes the current state of home care services in Reykjavik. Through this, we aim to identify areas for improvement in the service and organizational structure, thus facilitating the seamless integration of home care services and digital technologies.
Results and Impact: Preliminary findings demonstrate promising results, including increased efficiency, improved patient outcomes, and enhanced user satisfaction. The impact assessment spans beyond technological aspects to encompass the broader implications for the well-being of individuals receiving home care services.
Learning for the International Audience: Our experience in Reykjavik, marked by the integration of digital welfare solutions into home care, offers valuable insights for an international audience seeking to enhance their own integrated care systems. We share lessons learned, successful strategies, and challenges faced during the implementation of technologies in the home care sector.
Next Steps: As we approach the conclusion of our project, we aim to present actionable recommendations for advancing the integration of home care services in Reykjavik and beyond. This includes refining digital solutions, optimizing organizational structures, and fostering continued collaboration between healthcare providers, technology experts, and the community. We look forward to sharing our findings and discussing the future of integrated care at the ICIC 2024 conference.
