Abstract
The pilot with the Shared Medical Appointment (SMA)started in januari 2021 in the Schilderswijk which is a deprived area of the Hague , with low health literacy and high prevelence of metabolic syndrome . This group of patients are are seen in the SMA together with a multidisciplinair Team (MDT) of professionals and scientists.
The patient journey of the participants starts at population care towards customized patient orientated care.
This proactive, integrative towards personalized care migth benefit applying a quadruple or quintuple aim.
The study desing is a quasi experimental population based controlled cohort trial
The study employs a dif -in -dif design using a regional control group on non participating GP ‘s
Good collaboration is important between the primary care , the social care , welvare , and science.
A shared shavings contract could proved financial incentives to realize efficiency gain value based competition.
The SMA will be evaluated in 2023-2024 to measure the effects of the Sma intervention.
