Abstract
Case study of a health care center in Merchtem: A bridge between healthcare and welfare and a search for a different way of working to achieve a high quality and integrated care for our patients.
Introduction: The jobs of many healthcare workers has become extremely challenging. Consequences are that the drop out rate of healthcare workers is increasing and the feasibility to achieve qualitative care for patients becomes very difficult. With this project we are trying out various ways of working together with different types of healthcare workers, we are looking for tools to facilitate these types of collaboration and we try to create a sustainable structural and financial system.
