Abstract
There are 60 Primary Care Zones in Flanders where different partners from the health and welfare sector work together. The goal is to help people with care or assistance needs in the best possible way.
Our Primary Care Zone works for 3 municipalities being Puurs-Sint-Amands, Bornem and Willebroek.
We are convinced that we can only help people with tailor-made care when the care providers work together more smoothly and better. So we know where to go with a care question.
To stimulate cooperation and integrated care, our Primary Care Zone organizes 3 speed dating events every year with health and welfare professionals. With these speed dates, professionals get to know each other. In this way, we encourage warm referrals and cooperation between different professional groups.
