The European VIGOUR project - Lessons learned on our way towards better integrated care

Abstract

Summary: Available evidence suggests that integrated care is unlikely to evolve as a natural response to emerging care needs in any system of care, whether it is planned or market driven. The VIGOUR project therefore supports 16 care authorities in progressing on their specific journey towards achieving better joined-up care. A context sensitive support programme has been designed to guide each of them through a staged process of ambition focussing, maturity-self assessment and operational implementation planning. Different resulting integration approaches are being piloted under every-day-conditions, with a view to progressing the transformation of current care practices towards sustainable models of integrated care. Against the background of hands-on experiences gained by individual pilot regions, this workshop aims at discussing lessons learned from the VIGOUR overall approach and its applicability beyond the project.

Background: Any care model development is strongly context-bound. The implementation of structural changes in every-day care delivery has therefore often proved difficult. The VIGOUR support programme aims at taking individual care authorities from where they currently stand and support them in taking the next step on their journey towards integrated care.

Aims and Objectives: One aspect that complicates the introduction of better joined-up care provision is the fact that integrated care is a ""nested"" concept rather than a predefined organisational model. The operational implementation of such a concept is always context-dependent and can take different forms. The aim of the workshop is to discuss with the participants what steps the individual VIGOUR regions have taken in developing and planning their integration approach and what practical experiences they have had. Furthermore, it will be discussed to what extent the VIGOUR process and the methodological tools used in this context may also be helpful for others.
