Abstract
Introduction: In this workshop, we present and discuss perspectives on cross-sectorial patient pathways and hospital admissions based on findings from two research projects. We focus on the definitions and categories involved in policy-making aimed at reducing the number and length of hospital admissions. In the workshop, we wish to present and discuss these perspectives with the audience in order to develop a cross-disciplinary dialogue.
Background: Demographic changes and an increased pressure on health care systems attract much attention to suboptimal patient pathways with repeated hospital admissions in research and policy-making nationally and internationally. There is a need to address the perspectives of the patients and the healthcare providers to develop supportive initiatives strengthening cross-sectoral patient pathways.
Aims/objectives: The aim of this workshop is to present and discuss discrepancies between the logics of a quality indicator, such as “avoidable hospital admissions”, and the logics involved in the daily work of the healthcare providers based on findings from two Danish research projects.
Target audience: Researchers, healthcare professionals, patients and policy makers in all sectors interested in developing optimal patient pathways.
Key learnings: Participants in the workshop will be involved in discussing new perspectives and insights on cross-sectorial patient pathways with the aim of creating ideas to new research projects and suggestions for how to reorganize clinical practice.

Cross-sectoral interventions - perspectives of primary healthcare providers by Cecilie Nørby Lyhne (15 minutes)
The work of being a patient - a project following the patient trail of repeated admissions, by Anna Louise Skovgaard (15 minutes)
Group discussion moderated by the authors. Groups of 6-15 participants are given a subject related to the cross-sectorial patient pathways to reflect upon and discuss. (20 min)
Each group will be asked to sum up three key points from their reflection, which will be presented and discussed in plenum (15 min)
