Transfer and adoption of digitally enabled integrated person-centred care good practices: which challenges and key elements need to be addressed for a successful transfer?

Abstract

Summary: This workshop explores the challenges and key elements to be addressed when transferring Good Practices (GP) to heterogeneous health systems with different needs, aims and scopes.

It features the core elements of the four GP of JADECARE (Joint Action on implementation of Digitally Enabled integrated person-centred CARE), concerning integrated care, chronic conditions, self-care, prevention and population health that are being transferred and adapted to the contexts of 23 Next Adopters (NA). GP are: Basque Health strategy in ageing and chronicity: integrated care (Spain), Catalan open innovation hub on ICT-supported integrated care services for chronic patients (Spain), OptiMedis Model-Population-based integrated care (Germany), Digital roadmap towards an integrated health care sector (Denmark).

It will also highlight the experience of two NA: Portugal and Estonia.

Background: The aging population and the burden of chronic conditions is increasing the demand for more comprehensive, efficient and smarter care. JADECARE intends to reinforce the capacity of health authorities to successfully address the transition to digitally enabled integrated person-centred care.

Aim/Objectives: Explore challenges and key elements when transferring GP to heterogeneous contexts.

Objectives:

-Introduce JADECARE to the audience.

-Share the Core Features (CF) of GP to be transferred

-Present the JADECARE transfer and implementation strategy

-Promote the discussion among speakers and attendees to highlight the challenges and key elements to enable the transfer of GPs

Target audience: Policy makers, decision makers, health managers, health authorities, health and social care professionals, researchers, academia, health experts, and the staff working on implementation of integrated care, digitalization and/or evaluation.

 

Facilitators/speakers:

Jon Txarramendieta and Yhasmine Hamu (Kronikgune Institute for Health Services Research) will introduce the workshop, moderate and facilitate the group discussions.

Igor Zabala (Osakidetza Basque Health service), Josep Roca (August Pi i Sunyer Biomedical Research Institute), Manfred Zahorka (Optimedis AG), Kuno Strand Kudajewski (Health Innovation centre of Southern Denmark) will describe the main traits of the CF of their GPs, highlighting the transfer challenges.

Vanessa Ribeiro (Administration of the Health System, Portugal) and Mart Kull (Viljandi Hospital, Estonia) representing two NAs will present their Local interventions and Action Plans to articulate the transfer during JADECARE.

All speakers will participate in a group discussion. Moderators will launch questions to speakers, and attendees will be encouraged to make contributions in post-its and attach them in posters/murals.
