Abstract
Summary: In this workshop, we introduce logic model as a tool to guide improvement planning within the context of integrated care and build participant capacity in incorporating this tool in their work. Logic models provides an overview of project components outlining activities planned to result intended, including the longer-term outcomes envisioned. Among other benefits, the process of constructing a logic model can create a shared understanding among stakeholders and the end-product can bridge gaps between strategic and operational goals. While resources on the construction of logic models are widely available, we aim to close the gap in practical implementation of logic models by sharing challenges experienced in case studies presented by our regional partners. Finally, the bulk of remaining workshop time will be for participants to brainstorm and construct a logic model with specific focus on linking planned activities with intended results.
Background: SCIROCCO Exchange is an EU-funded project that involves 9 European regions across Europe to scale-up integrated care initiatives within respective health system contexts through facilitating knowledge transfers and providing personalized improvement support. We used logic models as a tool to facilitate the improvement process in regional integrated care initiatives.
Aim: Introduce logic model as a tool to guide improvement planning within the context of integrated care and build participant capacity in incorporating this tool in their work.
Objective:
-To prepare the group in discussing different components of integrated care and chal-lenge the causal relations and underlying assumptions
-To understand the role and benefits of logic model use in integrated care planning, implementation, and improvement.
-To collaborate with a broader set of stakeholders to jointly construct a mini logic model and use that to communicate the program to others
Target audience: Staff working directly in the implementation of integrated care systems would benefit from this workshop. Integrated care systems at different stages of maturity – from initial planning stages to implementation, and evaluation. It is advised that teams jointly participate in this workshop to maximize learning and benefit.
Speakers: Specific roles during the workshop include Presenters, facilitators (guiding discussions), time-keeper, note-taker (of workshop outputs). The exact roles and the number of facilitators will be adapted according to the number of workshop participants
