Co-designing integrated value-based eHealth technology for people living with frailty in Ireland

Abstract

Healthy ageing has become a global health priority as life expectancy is increasingly improving. With longer life spans, people are more at risk to experience health conditions which, in turn, places greater demands on our current health and social services. The ValueCare project is a Horizon 2020 European project which aims to deliver efficient outcome-based integrated health and social care to older people with chronic health conditions in order to improve their quality of life, as well as their families and to support the sustainability of healthcare systems in Europe. In Ireland, the aim of the ValueCare project is to co-design integrated value-based eHealth technology to support older people (≥75) experiencing mild to moderate frailty and their family caregivers to live independently in their homes. A two-phase approach will be adopted. The first phase involves a co-design process in which we will work together with older people living with frailty, family caregivers, health and social care professionals and health and social care managers to design and create the eHealth technology. The second phase will involve a pilot test of the technology in Cork and Kerry health and social care settings which will support the care pathway of the Integrated Care Programme for Older People. Our research seeks to identify a novel and scalable eHealth technology that can be translated into health and social care practice to support the needs of older people and address the complexities in the provision of their care.
