Abstract
The value-based healthcare initiatives in New South Wales (NSW) have been realigned in order to better coordinate the efficient management of chronic conditions across the continuum of care.
NSW, as a leader in Australian health policy areas of integration and value, developed several initiatives to bridge system gaps and maximise innovation. For people with complex and chronic conditions, the three approaches of Leading Better Value Care (evidence based clinical pathways), Integrated Care (chronic care models) and Collaborative Commissioning (partnership commissioning) have driven improvements in patient centred care.
Following the release of the NSW Future of Health Strategy, and guided by feedback from patients, care providers and senior system leaders, NSW has unified the maturing policy approaches under a single governance. This ‘one system’ approach for policy makers and service providers enables NSW to further develop a whole of patient journey infrastructure of evaluation, informatics and reporting to realise the NSW Quadruple aim of value-based health care for integrated care. For patients, the integration of these initiatives will provide a more efficient management of complex and chronic conditions and coordinated transfer of care across the system.
