Stress-Induced Takotsubo Cardiomyopathy Triggered by Awake Fiberoptic Intubation: A Case Report

Takotsubo cardiomyopathy (TC), also referred to as stress-induced cardiomyopathy or “broken heart syndrome,” is a transient cardiac condition characterized by reversible left ventricular dysfunction, most commonly seen in postmenopausal women after physical or emotional stress. We report the case of a 60-year-old postmenopausal woman who developed sudden cardiovascular collapse following awake fiberoptic nasal intubation for elective excision of a large orocervical mass. Transthoracic echocardiography revealed classical apical ballooning with preserved basal contractility and moderately reduced ejection fraction. Prompt cardiopulmonary resuscitation and hemodynamic support resulted in a successful return of spontaneous circulation. This case highlights the importance of vigilance in high-risk individuals, early recognition, timely resuscitation, and providing comprehensive supportive care for a favorable outcome.
