We are pleased to present this special issue marking Houston Methodist’s 40-year milestone of performing heart transplants. While Dr. Michael DeBakey was among the earliest pioneers to attempt heart transplantation in the late 1960s, he paused his efforts until 1984 when the immunosuppressive agent cyclosporine provided a path to consistently safe and successful heart transplantation. In the 40 years since, almost 100,000 heart transplants have occurred in the United States, bringing new hope to patients who faced limited life expectancy from heart disease.
Against a backdrop of historic development, the articles included here explore both the challenges and advances of heart transplantation from the beginning to the current day. Our authors share insights on the shortages of donor organs, the complexity of immunosuppressive therapies, and the continual quest to improve patient outcomes. They also explain the impact of today’s challenges on future efforts, emphasizing the importance of ongoing innovation, vigilant consideration of policy, and multidisciplinary collaboration to achieve ever-better clinical outcomes and ensure equitable access to this life-saving therapy. Looking back at how far we’ve come and envisioning amazing opportunities for the future, I’m inspired with hope for what we will achieve by our 50th anniversary—and the forty years to follow!
