Integrating care for children and young people – making and measuring impact

Background:This proposed workshop will support participants to consider what integration is, why it matters, and explore ideas for addressing fragmentation in the systems they operate in. The session aligns with the conference’s theme of “improved outcomes through community-based care”.

We consider it vital to pursue these aims in relation to care of children and young people (CYP). The discussion and learning will be applicable beyond CYP contexts.

CYP and families have been involved in the design and evaluation of example models.

Audience: We anticipated audience would be a range of managerial, clinical, academic and other professionals from the UK and abroad. This would include paediatric medical and adult medical teams, for whom the discussions and learning would be transferable.

Approach: This coached workshop would build on illustrative examples of effective integration of healthcare for children and young people in two areas of London. These provide context and experience as well as evidence of how integration can improve health and enhance equity.

Outcomes: We expect participants will consider the environments they operate in, generate ideas for how to integrate aspects of care, and takeaway practical ideas around impact and measurement.
