Insights from North America’s First Compendium of Integrated Care and Opportunities for Spread

Background: Integrated care initiatives are often developed in isolation, limiting opportunities for shared learning and collective impact. To address this, the North American Centre on Integrated Care (NACIC), in partnership with IFIC Canada, created the first North American Compendium of Integrated Care, as a national sight line of integrated care and population health initiatives so that it is a reference point designed to foster community, build capability, and accelerate transformation.

This compendium is unique in its scope and structure, using a standardized, internationally validated template to capture key dimensions of integrated care, including program design, target populations, care coordination, care teams, transitions, data sharing, program maturity, policy innovation, and, critically, patient and caregiver partnership It is also the first tool of its kind to explicitly include the perspectives of people with lived experience (PWLE) as partners.

The compendium will grow over time, enabling knowledge mobilization and impact across jurisdictions in North America. This workshop will showcase the compendium, including a demonstration of the tool, share early insights, and explore opportunities for spread, scale, and global collaboration to advance integrated care and population health.

- Service providers, clients, caregivers, and community members

-International delegates seeking scalable insights

Participants will include those who influence system design and implementation and are positioned to adapt and spread proven practices in their own contexts.

• Feedback on the online platform: usability, completeness, accessibility – strengths and how it could be improved
