Introducing a novel Hospital at Home Capability Maturity Framework (HaH CMF) and HaH Deployment at Royal Hospital Donnybrook

Abstract

Hospital at Home (HaH) will be an increasingly important feature of future health systems and the shift from hospital to home is one of the ten Copernican Shifts underpinning the new theory of the business for Health Stay Left, Shift Left-10X. Hospital at Home is made possible by advancements in digital and networking technology such as the Internet of Things (IOT), Cloud Computing and Advanced Wireless/Cellular Technologies. To assist collaborators in Guided Open Collaborative Ecosystems develop, deploy and use Hospital at Home Solutions a Hospital at Home Capability Maturity Framework (HaH CMF) has been developed and then used to guide the deployment and adoption of an advanced hospital at home solution at the Royal Hospital Donnybrook, Ireland. Capability Maturity Frameworks are a breakthrough management tool which organize information, processes, metrics and practices in an evolutionary maturity curve to help organizations advance and improve their capability in a particular domain. The HaH CMF organizes information and solutions in five maturity levels old practice, current practice, best practice, next practice and finally optimum practice. The HaH CMF describes the key characteristics of each level and how it can be used to help organizations advance their HaH capability. The paper also describes key trends and research which shaped the development of the HaH CMF.

 

In addition, this paper describes the development and deployment of a Level 4 (next practice) Hospital at Home solution at the Royal Hospital Donnybrook using a co-innovation approach and also shares the evaluation criteria used to select a co-design partner. Using the selected Redzinc Blue solution, a digital HaH solution was co-designed and instantiated to allow early discharge of Stroke patients allowing patients to better recuperate at home and also freeing up hospital bed capacity for sicker patients. The HaH solution featured novel functionality such as real-time remote vital signs, one click secure Telehealth and Virtual Ward Dashboards as well as FHIR integration to the Hospital EHR VCare. The presentation also shares the feedback from the early patient adopters and how their inputs were used to quickly iterate the HaH solution for broader Living Lab Proof of Concept Deployment.
