Beyond Burden: Integrating Psychosocial Support to Fortify Caregivers and Stabilize Long-Term Dementia Care

Abstract

Background: The progressive loss of autonomy in persons with dementia (PwD) creates a substantial, multi-faceted burden on their informal caregivers. Beyond instrumental support, caregivers require dedicated tools for disease comprehension and self-care. Unmet emotional and informational needs across the disease trajectory are key drivers of exponential caregiver burden, which directly escalates the risk of burnout and, ultimately, the premature institutionalization of the PwD. Addressing these needs is a critical imperative for successful integrated care systems.

 

Methods: Recognizing this critical need, a Catalan intermediate care center developed a tailored Caregiver Support Program. The program's content was co-designed and ensured for relevance through initial individualized interviews and focus groups with 50 caregivers of PwD.

The multidisciplinary team includes a specialized psychologist for emotional support, alongside a geriatrician, neuropsychologist, social worker, geriatric nurse, and occupational therapist. The program is applicable to caregivers of PwD in both the community and in psychogeriatric long-stay units.

 

The intervention is highly flexible, structured around three main, needs-tailored options: 1) Socio-educational Group Sessions (6 x 90-minute hybrid sessions) providing information on dementia management, autonomy promotion, and health; 2) Therapeutic Support Groups (weekly for three months) offering a structured space for shared experience and emotional healing; and 3) Individualized Emotional Support Sessions (up to 8 sessions) led by the psychologist. Key variables registered include emotional burden (Zarit Test), perceived Quality of Life (EuroQol-5D), and satisfaction.

 

Results: Over 200 caregivers participated in the program during 2024. Participants reported near-total satisfaction with the socio-educational sessions, particularly valuing the content's applicability and the positive impact on their sense of self-efficacy.

While overall group-level Zarit and EuroQol-5D scores showed stability, the more intensive, individual-focused interventions (therapeutic groups and individual sessions) demonstrated measurable clinical improvement: a mild improvement in emotional burden was seen in 52% of participants, improved Quality of Life in 40%, and reduced anxiety in 14%. The overall satisfaction level with all program interventions was reported as very high.

 

Implications: Key Learning Points and relevance for international delegates

The implementation of dedicated, tailored psychosocial and educational support, integrated into intermediate care, successfully addresses the multi-faceted needs of dementia caregivers.

Integrated Psychosocial Care is Essential: Findings confirm that specific emotional support (e.g., individual therapy) is crucial, leading to measurable improvements in emotional burden and QoL that surpass the impact of educational sessions alone.

 

Caregiver Support = System Stability: The model proves that fortifying caregivers is a high-impact, cost-effective strategy that actively contributes to preventing the premature institutionalization of PwD, thereby increasing system stability.

Co-Design & Transferability: Involving caregivers as co-designers ensured relevance and high satisfaction, leading to optimal resource allocation. The model’s effectiveness across both community and institutional settings validates its flexible design for broad application in various intermediate and transitional care services globally.
