Transforming care for Complex Brain Disorders through multidisciplinary teams at the Brain Medicine Clinic in Toronto, Canada

The Brain Medicine Clinic (BMC) at the Sunnybrook Health Sciences Centre is a novel clinical model established in 2020 that facilitates the integration of perspectives and skills from multiple specialties (e.g., geriatrics, psychiatry, neurology, paediatrics, and physical medicine & rehabilitation) to deliver timely diagnosis and collaborative holistic care for patients with complex brain disorders. The BMP pathway of care is supported by a neuropsychologist and patient navigator, an embedded social worker who supports patients and caregivers to identify and access personalized education and mental health support. This care model prioritizes collaboration across clinical sub-specialties and allied health professions to provide timely access to health services and improve quality of care for patients with CBDs and their care partners. Our results provide emerging evidence that this integrated brain medicine approach can reduce fragmentation and inefficiencies in care pathways. This session will explore how this bottom-up, clinician led initiative has fostered the identification of new approaches and policy solutions to advance effective, sustainable, and equitable health systems responsiveness for people with complex brain disorders across Ontario, Canada.
